Mehdi Fonoonizadeh was born in Tehran after his parents came from Ardabil.
He started his football in Rah Ahan in the south of Tehran but played for Nirooye Zamini, Daraei, Bank Tejarat, Esteghlal, Saipa and finally Pas. In Bank Tejarat he played with Ali Daei before Daei left for Persepolis.
Fonoonizadeh won the 1990 Asian Games Gold Medal in Beijing and for Esteghlal Tehran, he won the 1991 Asian Club Cup.
He is best known for scoring two long-range goals against Saudi Arabia in FIFA World Cup 1994 qualifiers in Doha. Morteza Fanunizadeh, his older brother, was Mehdi's teammate in the national team, and from 1990 to 1992, Mehdi played for Esteghlal and Morteza played for rivals Persepolis in Tehran.
He played as a sweeper and in midfield.
